I hereby give permission for distribution by:
1) GRUPA TRIP Sp. z o.o. based in Zakopane (Grupa TRIP)
2) Grupa TRIP Sp. z o.o., located in Zakopane, 21 Sienkiewicza Street
my image captured in the form of videos or photos in connection with the Tango Infinity Festival from March 19-23, 2026 called Tango Infinity Festival exclusively for the purpose of promoting the event.
1) This permission is free of charge and is not limited in quantity or territory.
2) My image may be used in various forms of electronic processing, cropping and composition, as well as combined with images of others, supplemented with accompanying commentary (e.g., name, place of work, experience), added to other materials without obligation to accept the final product.
3) This permission covers the following places of publication:
a) publication on the Tango Infinity Festival YouTube channel.
b) publication on the Tango Infinity Festival LinkedIn profile
c) publication on the Tango Infinity Festival website and social media.
4. The image will not be disseminated by the co-organizers for purposes other than those described above. Any other forms of promotion will be agreed and confirmed by email.
5. The image will not be used in an offensive form or publication or in any other way violate my personal rights.
6. This permission is given indefinitely

* I hereby agree to receive marketing notices from Grupa TRIP Sp. z o.o. regarding events organised by Grupa TRIP Sp. z o.o., including conferences, seminars, workshops and other promotional initiatives, to the e-mail address I provided. I declare that I have been informed about the possibility to withdraw the submitted consent at any time and the withdrawal of the consent does not affect the compliance of the processing performed before the date of its withdrawal.

You have the right to withdraw the consent you have given at any time. Entities performing activities for the Administrator, e.g. a mail hosting provider and entities authorised on the basis of legal regulations will be the recipients of the personal data.

* Pursuant to the Act of 29 August 1997 on the protection of personal data (Journal of Laws of 2002, no. 101, item 926, as amended) and the Act of 18 July 2002 on the provision of electronic services (Journal of Laws of 2002, no. 144, item 1204), I hereby declare that I consent to the processing of the personal data provided by me by Grupa TRIP Sp. z o.o. at 21 Sienkiewicza St., 34-500, for purposes related to participation in the Tango Infinity Festival in Zakopane 03 – 07.04.2025.
I acknowledge that (a) the controller of my personal data is Grupa Trip Sp. z o.o., (b) my personal data will be processed for the purpose of operating the Portal and using its functions, (c) providing my personal data is voluntary, however, refusal to provide my personal data will result in the impossibility of registering on the Portal, (d) the processed data includes: name, surname, e-mail address, telephone number, (e) I have the right to access and correct my personal data.

* Participation in the Tango Infinity Festival and hotel room reservation is subject to the payment of the fee indicated in the confirmation of registration within 3 days from the date of registration. The payment can be made by
- bank transfer
- online payment / online payment link
Failure to pay on time may result in removal from the list of event participants. In case of cancellation of participation in the event, the paied amount will be deducted.
Delivery date - immediately upon receipt of payment. Confirmation of booking will be sent by email
Reservation & cancellation terms:
- up to 14 days after purchase – full refund
- later than 14 days after purchase and more than 60 days before event date – 50% cancellation fee
- later then 60 days before event date – 100% of the value of the order

GRUPA TRIP has the right to hold and use the received amount for the above-mentioned penalty charges. Should the sum of the prepayment exceed the penalty charges, GRUPA TRIP shall return the balance.
